Get ready to apply for an ESA! Please bring the following documents with you to ensure a smooth application process:

✔️ Parent/Guardian Information

• Government-issued photo ID (driver’s license, passport, or state ID)

• Parent’s Social Security Number (if required for verification)

✔️ Proof of Iowa Residency (must show current address such as one of the following)

• Recent utility bill (electric, water, internet, etc.)

• Lease or mortgage statement

• Iowa driver’s license with current address

✔️ Student Information

• Birth certificate (or other proof of age)

• Student’s Social Security Number (may be required for verification)

• Current school enrollment verification
